THESE TERMS AND CONDITIONS CONSTITUTE A LEGAL AGREEMENT BETWEEN YOU AND THE NOGA COMPANY, LLC. PLEASE READ THIS AGREEMENT IN ITS ENTIRETY BEFORE YOU CONTINUE TO REGISTER FOR ANY NOGA PROGRAM. BY REGISTERING, YOU AGREE TO BE BOUND BY THE TERMS AND CONDITIONS OF THIS AGREEMENT.

PARENTAL CONSENT:
My son/daughter is in good health and has my full permission to participate in a NOGA soccer camp. He/She has had no previous sickness, illness, disease, or bodily injury that is contradictory to participation. I understand that soccer is a contact sport, and that physical injury may occur during the course of practice or games. I give my full permission for all such diagnostic and medical procedures as may be deemed necessary by the examining physician for my son/daughter.

Permission is also granted for NOGA to use any individual video tape or photographs taken at the camp in conjunction with publicity.


PAYMENTS AND DEPOSITS
Payments may be made by check, Discover, Visa, MasterCard or AMEX. The price that you pay is entirely dependent upon when your final payment is made. The price scale is itemized on the NOGA Website and in the official NOGA Brochure. Please be aware that if you decide not to pay in full at the time of registration your final price will rise in line with the price scale. In all cases your final payment is due 14 days before the first day of camp.


PRICE
To receive the best price for camp and the largest saving you should register and pay in full as early as possible. To qualify for early registration payment must be received in full by January 31st. After Feb 1st you can pay in full and secure a place at any one of our academies with just a $125 deposit. The total cost of camp you will pay is that listed in this brochure when the balance is paid in full.


DISCOUNTS
For Academy Camps $20 discount will be awarded to: a) siblings attending camp, b) those attending more than one session.
A $20 team discount will be applied to each member of any group of 5 or more campers registering together either at the same time or in the same envelope.
Only one Gift Certificate is allowable per family.
A camper may only use one type of discount.
These discounts do not apply to Mini-camps.
Discounts cannot be combined with any other Special offer such as the Early Bird Special
Volume discounts for club sign up days or complete teams are also available. Please call our Head Office for further details.


REFUND POLICY:


TUITION PROTECTION PLAN:
We created the Tuition Protection Plan to protect families who risk seeing plans change and must cancel registrations. Tuition Protection must be subscribed to at the time of enrollment and may not be added at a later date.

The cost for Tuition Protection is as follows:

REFUND ELIGIBILTY
With the Tuition Protection Plan: (all refunds exclude tuition protection fee): Cancellations can be made at any time up to the first day of camp and result in a cash refund of all monies paid. After the start of camp; cancellation will result in cash refund of amount paid, less $125 deposit per week. Or you may select a NOGA camp credit refund of amount paid, usable through the 2007 camp season.
Without the Tuition Protection Plan: Cancellations prior to April 1, 2006 result in cash refund of amount paid, less $125. After April 1, 2006, cancellations at least 15 days prior to the start of the camp week result in camp credit refund of amount paid, less $125 per week, usable through the 2007 camp season. No refunds are issued for cancellations received within 15 days of the start of the camp week.
All requests for refunds must be made in writing via post attention NOGA Cancellation Dept. P.O. Box 262, Garden City NY 11530 or via email to cancel@nogasoccer.com or via Fax attention Cancellation Dept. to (516) 489-3970. All refunds will take 14 days to process.
NOGA Registrations are transferable. Transfers must be in writing and must be received no later than 15 days prior to the start of the camp week. No refunds are issued for no-shows, late arrivals, early withdrawals, absences, power failures or camper dismissals for disciplinary reasons. Falsifying information is grounds for dismissal without refund. We reserve the right to cancel a session date or location if participation numbers are not adequate. You may select an alternative session date or request a full cash refund of tuition paid.

INSURANCE:
Supplementary accident insurance is provided for each NOGA camper. This insurance covers only medical costs resulting from injury at camp which are not covered by your own medical insurance and which are evaluated as justifiable by the NOGA insurance company.

The vital information packet includes everything parents need to prepare for camp, including campus information and directions, medical/permission forms and general camp information. Packets are available online at www.Nogasoccer.com
